**To receive assistance, everyone must complete a coordinate entry survey. This can be done in one of two ways:**
1. **By calling 211 and completing the questionnaire over the phone.**
2. **By visiting PATH and filling out the survey with the help of a case manager.**
**Once the survey is completed, individuals will be placed on a queue based on their level of vulnerability. From this list, we will provide support in the following ways:**
– **Placement in our dorms**
– **Providing meals**
– **Offering clothing, showers, and laundry services**
– **Delivering hygiene products**
– **Providing case management to help overcome obstacles and achieve stable housing**
